Zoloft (sertraline), a popular antidepressant known as a “selective serotonin reuptake inhibitor” (SSRI), is linked to dangerous side effects in users and serious birth defects (including, heart murmur, hole in the heart and other serious heart defects) if taken by pregnant women. Since 2006, the FDA has warned against the use of Zoloft and other SSRIs in pregnant mothers.

Paxil (paroxetine), one of the most widely prescribed SSRI’s for depression in adults has also been associated with serious side effects in men and women and very dangerous birth defects like heart defects that can result in death if taken by pregnant women. Also part of the 2006 study of SSRI side effects, Paxil has been identified by the FDA to be dangerous for women who are pregnant or who become pregnant.

Prozac (fluoxetine) is probably the most popular anti-depressant and SSRI prescribed by physicians over the past decade and is considered by the FDA to be highly dangerous for women who are pregnant or may become pregnant. Side effects of Prozac include heart defects, cleft palate/cleft lip, and persistent pulmonary hypertension in newborns (PPHN).

Celexa (citalopram), one of the more recent brand-named SSRI’s used to treat depression, was identified by the FDA in 2011 as being linked to very serious side effects as well as birth defects. The FDA issued a warning to adults who took higher dosages of Celexa that they are at a greater risk of deadly heart conditions including fatal heart rhythm problems. Wellbutrin (bupropion hydrochloride), was identified by the FDA in 2009 to have links to dangerous side effects and potentially fatal birth defects. A public health advisory was released by the FDA pointing out that Wellbutrin, Chantix and Zyban may increase the risk of suicide and serious neuropsychiatric symptoms.

Lexapro (escitalopram oxalate) is prescribed by doctors to treat depression and generalized anxiety disorder and is in the group of medications classified as SSRIs or selective serotonin reuptake inhibitors. Similar to Zoloft, Paxil and Prozac, Lexapro has been linked to serious and dangerous side effects including thoughts of suicide in adults and heart defects in children from mothers who have taken these products.

Effexor (venlafaxine) is an antidepressant and anti-anxiety drug that was approved by the FDA and released on the market in 2004 as an another SSRI. Since then it has been shown to have a direct correlation with permanent and dangerous birth defects including abdominal birth defects, anal atresia, cardiac defects, limb defects and genital defects.

It is a deformity present at birth in which the bones in the ankle are incorrectly placed. When this happens, the sole of the foot faces inward and the toes point down.

    Symptoms

    In the typical clubfoot, the foot is bent downward and the ankle twisted so that the sole faces inward. In a few instances a doctor or physical therapist can manipulate the foot into the normal position. Such cases usually recover in a few weeks with a minimum of help. However in most cases the condition cannot be fully corrected so easily and will need more prolonged treatment. Occasionally the foot is deformed in the other direction – it is bent upward and the sole faces outward. Another type of deformity affects the front part of the foot only. It is important for a doctor to distinguish these problems as soon as after birth as possible. Without attention the deformity may progress and the child will develop an abnormal gait.

    Treatment

    The earlier treatment starts the better. In the first few weeks of life, the deformity may respond to fairly simple measures. Repeated gentle stretching of the foot will encourage it into the correct position while bandaging between stretches will help keep it there. Doctors sometimes use splints and plaster of paris to achieve a good position. When the child starts to walk, he or she may need special shoes to maintain the best foot position. When simple measures fail to correct the condition, surgery may be needed to prevent the deformity from becoming permanent.

    Craniosynostosis

Craniosynostosis refers to premature closure of the flat bones of the skull while the brain is still growing in size. This causes pressure on the brain.       Before birth, the fundamental site of gas exchange in the fetal circulation is across the placenta. Shunts (foramen ovale and ductus arteriosus) are present in the fetal circulation to allow for placental gas exchange. At birth, major circulatory changes occur with closure of the shunts and establishment of the lungs as the site of gas exchange. In some children, these shunts or holes dividing the right and left sides of the heart remain open, resulting in the following types of congenital heart defects: atrial septal defects, ventricular septal defects, and patent ductus arteriosus. The circulatory pressures required to pump the blood to the body (left side) are higher than those required to pump blood to the lungs (right side), thus creating in effect a left to right shunt of blood away from the body to the lungs. Less severe variations of these lesions can close spontaneously, whereas more severe variants may require special cardiac medication, catheterization, or surgery for closure.

      Persistent pulmonary hypertension

      Another dangerous side effect of Wellbutrin is persistent pulmonary hypertension. It means, literally, persistent high blood pressure in the lungs. While a baby is in the womb, the pressure on the blood vessels in the lungs is high and the vessels are narrow. Once the baby is delivered and takes his or her first breath, filling the lungs with air, the pressure in the blood vessels should fall and the vessels should expand, so that blood from the heart can flow into them. The changes in pressure and the new pattern of blood circulation should cause the two ducts, the ductus arteriosus and the foramen ovale, to close. If, for some reason, the pressure in the blood vessels of the lungs does not fall, too little blood flows to them. The foramen ovale is not pushed shut but continues to allow blood to flow from the right to the left side of the baby’s heart and through the ductus arteriosus as it did in the womb. As a result, much of the blood bypasses the pulmonary artery and does not travel to the lungs to pick up oxygen for the rest of the body. Unless the condition can be corrected by drugs or surgery, the baby becomes increasingly ill and short of oxygen, and is likely to develop heart failure.

              “In a statement Thursday, Glaxo said it expects to finalize the terms of the settlement next year and put an end to the company’s ‘most significant’ US liabilities,” the Washington Post (11/3, El Boghdady) reported. “Those include a seven-year investigation into the marketing of 10 popular Glaxo drugs, a Justice Department probe into possible Medicaid reimbursement fraud and another into the development and marketing of the diabetes drug Avandia [rosiglitazone].”

              The National Journal (11/3, Fox, Subscription Publication) pointed out, “The case is not completely settled, but the agreement brings the British drug company and the US government closer to resolving the issue.” Providing background on the story, the National Journal explained, “The US government has been trying to clamp down on drug companies’ illegal marketing practices, from promoting drugs for unapproved uses to sponsoring clinical trials designed to make drugs appear more effective than they actually are.”

              “Negotiations over the terms will be completed next year, the London-based company said in a statement” yesterday, Bloomberg News (11/3, Serafino, Kitamura) reported. “The cost is covered by existing legal provisions and will be paid from the company’s cash resources, Glaxo said.”

The letter followed a March inspection of a plant in Worthing West Sussex making the injectable antibiotic Timentin. The agency said that GSK had failed to establish procedures to prevent microbiological contamination of drug products, as a disinfectant used in the plant was unable to remove all types of bacteria. The company said that all Timentin batches have met release standards, adding that it has been working with the agency to address issues raised by the letter and would file a “comprehensive response” by month’s end.

                    Infant Heart Defects

                    Ingestion of Wellbutrin during pregnancy could cause infant heart defects, including hypoplastic left heart syndrome. Hypoplastic Left Heart Syndrome is a congenital heart defect. It involves a spectrum of severe heart defects with a common problem: the main pumping chamber of the heart – the one that sends blood with a new supply of oxygen to the organs of the body-has not developed adequately. In addition, part of the body’s main supply artery, the aorta, may be missing or underdeveloped.

                    Babies with this type of congenital heart defect can live for a time following birth as long as a connection between the normal right ventricle and the infant’s aorta remains open. That open normally closes hours to days following delivery. Medication can keep it open for weeks to months. Once the connection closes, the heart cannot deliver oxygenated blood to the tissues and the child will die.

                    The Norwood sequence of procedures is a serious of operations that make it possible for the healthy right ventricle which normally only pumps blood through the lungs and back to the heart to do the work that usually two ventricles working together provide. The first operation can take place in the first days of life. If successful, it will sustain the baby’s life for months and allow time for important growth and development. The operations do not afford complete correction of the heart. It may provide sufficient heart function to support average or better daily activity for many years but at some point, the single pumping chamber may fail to provide everything the body needs. The operation also carries considerable risk of morbidity and mortality.

                    An alternative to surgery is heart transplantation – the removal of the defective heart with replacement by one with normal structure and function. The baby must wait for a donor heart to become available. A large proportion of babies awaiting for a new heart die of complications of their disease and treatment while waiting for a new heart.
