    Although testosterone use has sharply increased among older men in the past decade, many patients appear to have normal testosterone levels and do not meet the clinical guidelines for treatment, according to new research accepted for publication in the Endocrine Society’s Journal of Clinical Endocrinology & Metabolism.

    Testosterone is a key male sex hormone involved in maintaining sex drive, sperm production and bone health. Since testosterone levels tend to naturally decline as men age, lower levels of the hormone do not necessarily mean that an individual has hypogonadism, a condition that results from low testosterone. As the population ages and an increasing number of men struggle with obesity and diabetes, more men may experience low testosterone levels without meeting diagnostic criteria or displaying symptoms of hypogonadism.

    “Over the past decade, older and middle-aged men are increasingly being tested for low testosterone levels and being prescribed testosterone medications, particularly in the United States,” said one of the study’s authors, J. Bradley Layton, PhD, of the University of North Carolina at Chapel Hill. “While direct-to-consumer advertising and the availability of convenient topical gels may be driving more men to seek treatment, our study suggests that many of those who start taking testosterone may not have a clear medical indication to do so.”

    To study testosterone trends, the retrospective incident user cohort study analyzed commercial and Medicare insurance claims from the United States and general practitioner health-care records from the United Kingdom during the period between 2000 and 2011. The study identified 410,019 American men and 6,858 U.K. men who began taking testosterone during this period. The analysis also found more than 1.1 million U.S. men and 66,000 U.K. men who had their testosterone levels tested during this time.

    Since 2000, the number of men beginning testosterone therapy has almost quadrupled in the United States while only increasing by a third in the United Kingdom. The majority of these patients had not had their testosterone levels measured recently or only had them tested once prior to beginning treatment.

    The Endocrine Society’s Clinical Practice Guidelines on testosterone therapy in adult menrecommend making a clinical diagnosis of androgen deficiency, or low testosterone, only in men with consistent symptoms and unequivocally low testosterone levels.

    While testosterone testing has increased substantially in the United States and the United Kingdom, the study found the testing appeared to be more targeted in the United Kingdom. Many of the tests there identified men who did have reduced levels of the hormone.

    “In the United States, we saw a clear trend where more and more men being tested actually had normal testosterone levels and non-specific symptoms,” Layton said. “This is cause for concern as research examines potential risks associated with testosterone use.”

Direct-to-consumer advertising is a strong driver of medical costs and it often promotes ineffective or dangerous products. It’s hard to convince patients that they were suckers of a sales pitch.
      2. The cost of one bottle, about one month’s supply, is quoted at more than $400.
      3. The risks from treatments are uncertain, but the list extends from prostate cancer to shortened life expectancy. Other concerns have included liver damage, overproduction of red cells, prostate enlargement, sleep apnea and worsening heart failure. A study of 8,709 men was published Nov. 6, 2013 in the Journal of the American Medical Association. The men in that study all had risks for vascular disease and testosterone levels below 300 nanograms per milliliter. The study was stopped because the testosterone-treated group had more heart attacks and strokes, and they also had a higher death rate — 27.7 percent compared to 19.9 percent in the untreated group.
      4. Doctors use average testosterone levels in young men as the standard for determining what is normal and what is not. It’s not reliable. Individuals vary and the response to treatment is different depending on what you measure: muscle mass, strength, sexual function and so on.
      5. There’s a lot we don’t understand about possible benefits and risks. The chemistry is complicated. For example, 80 percent of the female hormones in men arise from a chemical reaction with testosterone. More testosterone means more female hormones.
      6. I suspect the most important thing we have to learn is that the basic concept was a mistake, one we’ve made before. We haven’t seemed to benefit from history. What warnings did we have?

              In November 2013, the Journal of the American Medical Association reported findings that linked testosterone therapy side effects to death and serious heart problems. Researchers from Denver-based VA Eastern Colorado Health Care discovered hundreds of instances where men had suffered from death, stroke, and heart attacks following administration of testosterone therapy.  The study analyzed details of 1,223 men treated with the hormone enhancement medications and reported that 33 suffered from stroke, 23 were hospitalized, and 67 died within 18 months of having testosterone therapy.

              Researchers claimed that there was at least 29 percent increase in the risk of cardiovascular problems in men treated with testosterone therapy. Supported by a team of experts from the University of Texas, they studied the medical history of 9,000 male veterans who underwent angiography between 2005 and 2011 because of blockage in coronary artery. It was found that those used testosterone supplements had 30 percent higher rate of cardiovascular adverse events resulting in death, stroke, or heart attack. The study held aggressive promotion and off-label prescription of testosterone supplements responsible for increase in the risk of side effects.

              According to a 2013 Wall Street Journal report, a high risk of heart problems linked to testosterone therapy side effects forced researchers sponsored by the National Institutes of Health to call off a hormone treatment trial for older men in 2009.
